Kindergarten Registration

KINDERGARTEN REGISTRATION

YOU WILL NEED THE FOLLOWING :

• Original birth certificate with a raised seal showing that the child is five (5) years old on or before October 1st.
• Residency Certification Print Form
• 3 proofs of residency. (i.e. Mortgage Statement, Certificate of Occupancy, Homeowner's Insurance, Tax Bill, Signed and dated Lease, Deed, Gas or Electric bill.)
• Provide a copy of your child's immunization record, signed by a physician.
  Immunization dates must include month, day, year.

• Universal Child Health Record completed by a physician.  Print Form
